After Shepard assists Samara in her mission to destroy her daughter, Samara pledges her loyalty to Shepard and stays by her side to assist her in the mission to destroy the Collectors, despite the very low chance that she'll return from the mission alive.

Samara assists the squad as they storm the Collector base, and once there, they discover that the Collectors are creating a Reaper, a sentient alien machine. Reapers appear in inhabited space and eliminate all advanced species every 50,000 years- and the fact that the Collectors are building a new one means that they'll be returning soon.

Shepard and her team kill this Reaper before it's finished, and destroy the Collector's base. Against all odds, everyone comes back from this mission alive. Samara doesn't stay with Shepard long though; she leaves once their mission is complete, though she offers her help to Shepard in the future.

Six months later, the Reapers attack Earth, and then proceed to attack the homeworlds and colonies of every other species. However, Samara does not make contact with Shepard until they meet on the planet of Lesuss, where Shepard is investigating a distress call from a group of Asari commandos. Samara reveals that she came to the mostly-uninhabited planet because the only settlement there happens to be a monastery for Ardat-Yakshi; and the place where her two remaining daughters live.

Samara and Shepard discover that the Reapers have already taken over the monastery, and are using the Ardat-Yakshi to create extremely powerful monsters called Banshees. As they fight through the Reaper forces inside the monastery, they discover that one of Samara's daughters, Rila, is already indoctrinated and slowly becoming a Banshee.

The only way to eliminate the Banshees that are overtaking the monastery is to use a bomb left by the Asari commandos. Doing so would both destroy the monastery and help cripple the Reaper forces, even if it would only be a temporary setback for them. Rila agrees to help them by offering to detonate the bomb personally, even though it would kill her. But her death would mean that she wouldn't become a Banshee herself.

Samara silently agrees to this, and leaves Rila to detonate the bomb. Falere, who isn't indoctrinated, has to be dragged away, screaming and crying, from her sister. Samara, Shepard, and Falere all escape the monastery safely before the bomb goes off, destroying the building and everything inside.

Once they're all safe outside, Samara attempts to comfort a grieving Falere. She tells her that seeing her daughters again reminded her of what was important, reminded her of why she swore she would lay down her life.

She draws a gun, and calmly says that what her code says about Ardat-Yakshi is absolute; they are only permitted to live if they remain in the monastery, and now there is nowhere for Falere to go. She declares that there is only one way to save Falere. Despite her daughter's protests, she puts her gun to her own temple and serenely pulls the trigger.

And that's where Samara's life ends. Unable fulfill her code, and kill her last living daughter, she chooses instead to kill herself. Falere is not a complete monster like Morinth, and Samara can't bear the thought of having to kill her, but doing so, she violates her own code. And she has said before, without her code, she is unjust, and cannot allow herself to live.

Samara does not change much in the six or so months between the time she leaves Shepard and the incident at the monastery. She has been alive nearly a thousand years, and would not have much of a personality shift in that time. She has, however, seen the Reapers invade her home planet and many others, and she has seen how hopeless the fight against them seems. But until she ends her life on Lesuss, she is fighting until the end.

When Samara returns to the City, she will have a new human body in addition to her usual Asari form. She can switch between them at will.
